Insurance Information
Washington State does have DIRECT ACCESS, which means you can go directly to a physical therapist for treatment.
Usually, insurance companies require a referral/prescription from a doctor.
Our fees are usual and customary for the area. All charges are based on what modalities and procedures are done at the time of service. We have preferred provider contracts with many insurance companies and abide by their payment rules. When your health plan permits, massage can be part of your treatment program.
Auto Injuries: Must have open Personal Injury Protection (PIP) plan. No Third Party Claims, no Settlement claims.
Work Injuries: Must have open claim or backup personal medical insurance.
We do not accept Medicaid/DSHS/Coupons.
Many insurance companies we are contracted with are not listed. If you do not see your insurance company listed above call our billing department, at (425) 672-2910 to see if we are a provider for your plan.
As a courtesy, Axis Physical Therapy will do their very best to verify and obtain accurate benefit information from your insurance carrier. We highly encourage our patients to be involved in all aspects of their health care including contacting their insurance carrier to verify the information we have obtained is accurate, as the information reported to our office is not a guarantee from your insurance carrier regarding what your final benefits/payments will be. Ultimately the financial responsibility of your health care belongs to you, the patient. Additionally, the insurance carrier has a fiduciary responsibility to you as the subscriber/customer allowing you more rights in managing a correction of misquoted benefits than we, as a provider of your service, are allowed/granted. Any accident policies (such as Personal Injury Protection) are an arrangement between the patient and the insurance carrier. Axis Physical Therapy is not a party to that contract. Our office will prepare any necessary reports and forms for processing of insurance claims. The patient is responsible for providing current and accurate insurance coverage information.
